Your First Day

Paid Intern Information Forms

Please bring the following checked forms with you on your first day of employment to facilitate the induction process and ensure that your Personnel File is complete and accurate.  Please complete each indicated item prior to your first day of employment.  Some of the forms are enclosed with the appointment letters; the other forms can be found below.

Required for your first day

Please bring the following information with you on your first day:

  • Original United States Social Security Card (Used to verify employment eligibility)
  • A local address (P.O. Box not acceptable for tax purposes)
  • A copy of your checking account/savings account number and your financial institution's 9-digit ABA number (i.e., voided check), or open an account with the FRB Federal Credit Union. (The Board pays by direct deposit only.)
  • Verification of degree requirement completion (i.e., transcript or letter from school registrar, if not previously submitted.)

The following forms must be completed prior to your first day of employment for submission during orientation.

  • Standard Form 86 (20 KB PDF) (SF-86)
    The Board's Personnel Security Office will send you an email message with a link to this form. Please note that e-QIP is not compatible with Apple computers. Once you've entered the website, please read the "Quick Reference Guide for the Applicant," before getting started. (This on-line form must be completed in its entirety, including 10 years of employment and residence information)
  • OR

  • Standard Form 85P (25 KB PDF) (SF-85P) The Board's Personnel Security Office will send you an email message with a link to this form. Please note that e-QIP is not compatible with Apple computers. Once you've entered the website, please read the "Quick Reference Guide for the Applicant," before getting started. (This on-line form must be completed in its entirety, including 7 years of employment and residence information)

  • Employment Eligibility Verification (I-9 Form) Leaving the Board
    This form can be completed and printed on-line or downloaded and saved to be printed and completed by hand from the U.S. Citizenship and Immigration Services website.

    Please read the instructions before completing this form and review the last page for the list of acceptable documents that you will need to bring with you to establish identity and employment authoriztion. (This completed form and the acceptable documentation will be reviewed during orientation on the first day of employment)

  • Fingerprinting

  • EEO Self Identification of Disabilities (13 KB PDF) (FR1437) [optional]
    Accurate statistics on employees with disabilities at the Board are used to prepare required statistical reports for the Equal Employment Opportunity Commission, to justify employment of support personnel and the purchase of special equipment, and to plan building renovations. You are not obligated to complete this form, but we strongly recommend that you do.

  • New Employee Data Form (294 KB PDF) (FR1424)
    This form gives us your name, address, and emergency contact information.


  • New Employee Pre-Information Sheet (21 KB PDF)
    Fill out and print this form so you can get a password.

  • Federal Withholding (68 KB PDF) Leaving the Board(W-4)

    State Withholding (D-4, MW507, or VA-4)
    Fill out and print your State Tax Registration Form for the area in which you live.
  • Direct Deposit (17 KB PDF) (FR 37)
    This form is used to collect bank account information so that your pay can be automatically deposited.
